On August 15, 2014 at 4:00 PM, the Dodge County Sheriff’s Office responded to an injury automobile crash on CTH A at the intersection with Indian Hills Trail in the Town of Beaver Dam. Investigation revealed that a 2010 Honda hatchback was traveling south on Cth A and slowing to conduct a turn onto Indian Hills Trail. A tractor/trailer rig was following the Honda, but the tractor/trailer failed to slow for the Honda. The front of the tractor/trailer struck the rear of the Honda, forcing it into the northbound traffic lane where it struck a northbound Dodge passenger car. The driver of the tractor/trailer (Lee Henricksen, 67 years old, from Fox Lake) was uninjured in the crash. The driver of the Dodge (Jennifer Polenska, 31 years old, from Waupun) was transported to Beaver Dam Hospital and later transferred to UW Hospital with non-life threatening injuries. The driver of the Honda (Judith Haddad, 45 years old, from Lindenhurst, IL) sustained non-life threatening injuries and was flown from the scene by Flight for Life to UW Hospital. A passenger in the Honda (Ilan Hurtado, 9 years old, from Lindenhurst, IL) sustained severe injuries and was flown to UW Hospital by another Flight for Life helicopter. Assisting at the scene were Beaver Dam Police and Fire Departments, Beaver Dam and Horicon EMS, 2 Flight for Life helicopters and DCERT.
